9-12, Youth Council, Families, Local Schools, & Volunteers.

Every parent must fill out for Grades 6-12 youth the fillable forms: (see below for all forms)

-Annual Registration Youth Ministry Programs
-Medical Release Forms
-Photo & Videography Release Forms
Forms to Be Completed - by parents for their child(ren)
FOR YOUTH UNDER 18 working with Youth - form completedby parents if the youth is under 18 years of age

The Diocese of Calgary requires completion of this form for any person, 18 years and younger who will be work or volunteer with younger children. The Diocese, along with your parish thank you for offer of service.
